   Information for patients

   Please note that the services we provide is only after referral from your Doctor or Dentist. They would know who is the  most appropriate person who can provide the treatment  for you.

Additionally if you have health insurance, one of the requirements is that you are referred  by your Doctor or your  Dentist. 

 As a specialist unit:

·    we  can only see patients in the Jaw Face Clinic  after referral  by your own Dentist or Doctor.

·     Some   referral letters or cards are pre-printed . If you have been given one of these, you may find it more convenient to  contact the hospital of your choice to book the  appointment  yourself.

·   or Chelsfield Park Hospital  on telephone  01689 885905.

·   I consult at The Sloane Hospital, on Friday evenings : and Chelsfield Park Hospital on Saturday mornings  



When you attend for the initial consultation  

·   Please remember to bring your referral card or letter with you. 

·   We also need Information about your general health, including the routine medication you take, any allergies you have and your experience following  previous surgery under general anaesthesia.

·     Name and address of your medical GP  .

·     If you do have health insurance it would be advisable to contact them to obtain pre-authorisation. Please bring details of your insurance so that any accounts can be sent directly to them. 

·    If you do not have any health insurance please be aware that the investigations and treatment in the private sector can be quite expensive. For instance admission for removal of impacted wisdom tooth or teeth  under general anaesthetic  is in excess of £1850. 00.

·    We do also have facilities to carry out a certain  number of procedures  under local anaesthesia  as out patients in the jaw face Clinic   .These are listed below.
